1915(i) Home Based Services Program
Embracing Hearts Healthcare is proud to offer the 1915(i) Program — a Medicaid-supported initiative designed to help individuals with mental health or developmental disabilities receive the care they need in their own homes. This program provides personalized support services such as skill-building and daily living assistance, aimed at promoting independence, stability, and a higher quality of life. Through compassionate, individualized care, the 1915(i) Program ensures that each participant can thrive in the setting that feels most like home.

We offer an introductory “meet & greet” visit where we get to know you and your family to better understand the support you’re looking for. During this visit, a nurse, CNA, or DSP will also meet with you to discuss your expectations and answer any questions, so you feel confident and comfortable with their care. We’ll also take care of any necessary paperwork at this time to make the process smooth and straightforward.

The 1915(i) program

In-Home Nursing Respite
is provided for persons who require skilled nursing services, as prescribed by a physician or nurse practitioner, in the absence of the primary caregiver.

Home and Community Support
This service is generally provided a few hours per day to assist an eligible individual with activities such as bathing, meal preparation, eating, dressing, shopping, leisure activities, community integration, and light housekeeping.

In-Home Respite
Provides temporary, periodic relief to those persons normally providing care for the eligible person.
